Grade 3 provincial assessments and Grade 10 provincial examinations:
- The standard alternate formats are enlarged-print, Braille, and black-and-white.
- Alternate formats must be requested through TIENET.
- The TIENET Request for Alternate Formats (NSA and NSE) document must be finalized by the date below for all students requiring one of these formats as a documented adaptation.
- December 19 for first semester examinations
- April 1 grade 3 assessments and second semester examinations

- PowerSchool roles that have TIENET access to create/finalize this form are Resource Teachers, Resource Teachers with Guidance Component, and School Administrators.
Grade 6 and 8 provincial assessments:
- Formats that are available with a documented adaptation include enlarged print and Braille.
- Paper-based alternate formats should only be ordered if a student’s documented adaptation cannot be met digitally.

- Your contact information (phone and email):
- You may be contacted for clarification.
- Do not request alternate formats through TIENET for Grade 6 or 8 provincial assessments.
